Job Summary Acquire verbal consents from patients' families for monitoring services via telephone calls, ensuring clear communication and understanding. Job Description Job Description Acquire verbal consents from patients' families for monitoring services via telephone calls, ensuring clear communication and understanding. Maintain meticulous records of all interactions and consents in accordance with company and regulatory guidelines. Collaborate with clinical teams to provide feedback and insights from readmission reviews and patient interactions. Accurately update the patient portal following each consent obtained, maintaining a high standard of data integrity. Proficiently navigate the Electronic Health Record (EHR) system, PointClickCare, to extract data pertaining to patient transfers. perform detailed readmission reviews (transfer out log analyses) by examining transfer logs from skilled nursing facilities Analyze and interpret data within the portal for all patients listed in the transfer out log, identifying significant clinical events. Create detailed and engaging presentations using Google Slides to showcase successful patient case studies demonstrating the impact of services. Generate compelling case studies that highlight instances where the monitoring services have successfully identified and mitigated potential hospitalizations. Maintain consistent and effective communication with the team, providing regular updates and reports on task progress and any insights gained. Qualifications Demonstrable experience in handling sensitive patient information with confidentiality and compliance with healthcare regulations. Strong oral and written communication skills, with the ability to effectively articulate consent information to patients' families. Competence in utilizing Google Suite, especially Google Slides, for creating professional and informative case study presentations. Ability to coordinate and communicate with teams, ensuring transparency and consistency in updates regarding task completion. Capable of working independently with a high degree of reliability and accountability. Excellent interpersonal skills and communication skills in English, both written and verbal Strong ability to handle multiple tasks and work effectively in a fast-paced startup environment Superior organizational and time management skills Nice to have Proficiency in navigating and managing data within EHR systems, particularly PointClickCare. Experience in healthcare or clinical settings, particularly in patient engagement or care coordination roles. Familiarity with telehealth and remote patient monitoring systems. Advanced data analysis skills, with the ability to synthesize complex information into actionable insights. Previous experience creating case studies or other educational materials in a healthcare context. Strong understanding of medical terminology and clinical workflows. Cross-cultural competency, including the ability to work effectively with diverse populations and understanding of U.S. healthcare practices, to better interact with the Health team and its patients' families Additional Information Must be willing to work in any Outsourced office - Alabang, Eastwood, Zeta Tower Bridgetowne, Cebu and Pampanga Work schedule: M to F, 11pm to 8am Manila Time Walk in to our Eastwood Office to fast-track your application.
