About the Job:We are looking for a dedicated Care Coordinator (LPN/RN) to join our client's healthcare team and support patient care through virtual and telephonic coordination. In this role, you will conduct patient assessments, develop care plans, and ensure continuity of care across providers and services. You will play a key role in patient education, monitoring progress, and identifying potential risks to improve health outcomes. This position offers the opportunity to deliver patient-centered care in a collaborative, technology-enabled environment.Key Responsibilities:
  • Conduct telephonic and virtual patient assessments and ongoing monitoring.
  • Develop, implement, and update individualized care plans based on patient needs and clinical guidelines.
  • Provide patient and family education on conditions, treatments, medications, and self-management strategies.
  • Monitor patient progress and escalate clinical concerns as needed.
  • Coordinate care across providers, payers, and community resources to ensure continuity of care.
  • Document all patient interactions accurately in electronic health records (EHR) and care management systems.
  • Collaborate with clinical and operational teams to enhance workflows and patient engagement.
  • Utilize clinical judgment to triage patient needs and support timely interventions.
Qualifications:
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN).
  • 1+ years of direct clinical experience (acute care, ambulatory, home health, or care management).
  • Experience providing care coordination for offshore or international patients
  • Strong understanding of hospital care coordination processes and clinical workflows.
  • Excellent interpersonal and communication skills (written and verbal) with the ability to engage patients remotely.
  • Proficiency in electronic health records (EHR), telehealth tools, and digital care platforms.
  • Knowledge of chronic disease management, preventive care, and patient education principles.
  • Bilingual skills (English/Spanish) are a plus but not required.
