● Communicate relevant updates to the healthcare team and escalate concerns as necessary.
● Review patient diagnoses to ensure accuracy and appropriateness of treatment plans.
● Conduct regular assessments of medical records and documentation for compliance with
established standards and protocols.
● Identify at-risk patients based on medical history, current condition, and other relevant factors.
● Perform comprehensive chart reviews to assess the adequacy of care and identify opportunities
for intervention or improvement.
● Collaborate with healthcare providers, clinicians, and administrative staff to identify
areas for improvement in documentation procedures and maintain consistent quality
across all records.
● Utilize extensive clinical knowledge and experience to assess patient needs, prioritize
interventions, and optimize outcomes.
● Review OASIS assessments and document recommended changes in the OASIS database tool
● Review ICD-10 coding and sequencing from documentation in the patient chart
● Enter results from review in appropriate fields of the database
● Provide explanation for change and reimbursement impact recommended Field Notes, Chart Review
● Be open to accepting new challenges and tasks as they arise.Requirements:
● Active Registered Nurse (RN) license.
● Excellent communication and interpersonal skills.
● At least 2 years of experience in US Healthcare Account
● Minimum of 2 years of clinical experience in a healthcare setting.
● Proficiency in medical terminology and electronic health records (EHR) systems.
● Experience in Homehealth or Hospice setting
● Effective communication skills, both verbal and written.
● Ability to work independently and collaboratively in a virtual environment.
● Excellent attention to detail and organizational skills.
● Strong analytical abilities with the capacity to critically evaluate medical information.
